Three Step LIFELINKS PROCess

1. Initial Assessment

Our Care Managers work with you and your aging loved one(s) to understand the entirety of your family's situation. We've done hundreds before, so we know how to read between the lines, ask the right questions, and listen to both what goes said and unsaid.

2. Care Plan

We take the initial assessment that our specialists and your family collaborated on to draft a holistic and comprehensive Care Plan for your loved one. It's a step-by-step plan that outlines the who, what, when, where, and how of your aging loved ones' care.

3. Care Plan Implementation

This is the core of what we do. We make sure that every aspect of your Care Plan is being executed as it should be. This can take many forms including but not limited to calling all family members to keep everyone on the same page, advocating for your loved one at doctor's appointments, and managing their medication.
